Sign/ Return/ Update Your Documents

As the rush of the first week back to school comes to an end and starts to slow down, please make sure you are all caught up with any and all paperwork that the school is requiring. A couple of documents you may need to know about or may still need to turn in:

The Front Office Stakeholder Agreement – From the ‘Welcome Packets’ given out at Kinder Kick-Off and Meet the Teacher. Each student, K-12, needs their own signed agreement turned into the front office.

Classroom Rules/Pillar of Character Agreement – Some classroom instructors provided their own classroom rules agreement that needs to be signed for each individual in that class, and returned to the instructor who requested it.

Birth Certificate – Please be sure you have a birth certificate on file for each of your students who attend EVCA. Whether you are new or returning, please be certain you have this document turned in to us in our files. Please email [email protected] to verify if you are unsure.

Immunization Records/Immunization Exempt Form – Please be sure you have each of your EVCA student’s Immunization records updated with our Health Office. Whether you are new or returning, please be certain this document is updated. Please email [email protected] to verify if you are unsure.

Student Parking Pass – Upper school students who drive themselves to school must have an updated parking pass for the 24-25 school year. This form was provided in all upper school ‘Welcome Packets’, but if you need one, please request a new one in the front office. The filled out forms, along with the documents requested on this form, need to be turned in to Mr. Rodriguez.

Student Athletic Early Release Form (Practice) – All student athletes, K-12, who need to leave school early on occasion or on a consistent basis, need to have their Student Athletic Early Release Form (Practice) filled out. These must be renewed for each student with every new sport, every new season. This form allows the student to check out between 2:45-3 for the duration of the sports season. Students who need to leave for sports games, please see the game form requirements below. Students should not need to leave earlier than 2:45 for practice. However, on the rare occasion that they should, the student athlete will need additional parent permission for each instance (either by note, by phone, or in person).

Student Athletic Early Release Form (Game) – All student athletes, K-12, who need to leave school early on occasion for a school sanctioned athletic game, need to have their Student Athletic Early Release Form (Game) filled out. These are required for each student for every single sports game in order to leave school early, if the parent didn’t provide a different form of permission of dismissal. The game form is meant to make everyone, office/student/instructor/parent, have an easier and smoother transition on game days. Your student will need to retrieve a blank form from the office (or print out at home) in advance to their game, get their form signed and filled out at home, and they will use this form to show their teacher when it is time for them to leave class, followed by showing the receptionist their form in order to leave school campus.
